LULING, La. -- A Thibodaux woman is facing charges after a crash took the life of a motorcyclist in St. Charles Parish Wednesday, according to State Police.

Around 8:45 p.m., Michael Cullins, 39, of Paradis, was driving north on a motorcycle on LA 52 when a van driven by Elke Forbes, 35, of Thibodaux, pulled out from a private drive on to LA 52 southbound, according to State Police, causing Cullins' motorcycle to slam into the van.

Cullins was taken to the hospital where he later died. Forbes and a passenger inside her van were taken to the hospital with minor injuries.

A blood sample was taken from Forbes, according to State Police, in order to determine if impairment was a factor in the crash, and the Louisiana State Police Crime Lab will conduct a toxicology analysis. Toxicology results are pending an autopsy on Cullins.

Forbes was arrested and charged with negligent homicide and failure to yield from a private drive.

The crash remains under investigation.
